ABSTRACT

Objective: To understand the infection status of mycobacterium tuberculosis among health workers in tuberculosis designated medical institutions and explore the risk factors of infection. Methods: From September 2021 to June 2022, a questionnaire survey was conducted among health workers in relevant departments of 4 tuberculosis designated medical institutions by cluster stratified sampling, including the implementation of hospital infection control measures in medical institutions and occupational exposure of medical staff to mycobacterium tuberculosis. Peripheral blood interferon gamma release assays (IGRAs) and lung imaging examination were performed to determine the mycobacterium tuberculosis infection. Factors with statistical significance in univariate analysis were included in multivariate logistic regression to analyze the risk factors of mycobacterium tuberculosis infection. Results: A total of 657 people completed the lung imaging examination and questionnaire, of which 654 people had peripheral blood IGRAs detection, and the latent infection rate of tuberculosis was 39.45% (258/654) . Univariate analysis showed that age, sex, marital status, economic income, occupational category, professional title, length of service, and other variables had statistical significances in tuberculosis latent infection (P<0.05) . In terms of personal health status, there were statistically significant differences in the distribution of health workers in terms of their tuberculosis history, tuberculosis history of their immediate family members, previous tuberculin skin test (TST) (P<0.05) . Multivariate analysis showed that there were four risk factors related to tuberculosis, including professional title (X(1)) , years of tuberculosis related works (X(2)) , tuberculosis history (X(3)) and previous TST (X(4)) . The regression equation of the probability of tuberculosis among health workers was y=-1.920+0.246X(1)+0.046X(2)+1.231X(3)+0.478X(4). Conclusion: The latent infection rate of tuberculosis among health workers in tuberculosis designated medical institutions is high. It is necessary to strengthen the management of infection control, carry out regular screening, enhance the self-protection awareness of health workers, and reduce their exposure to mycobacterium tuberculosis and infection risk.

ABSTRACT

OBJECTIVE: To explore the abnormal manifestations and clinical features of patients with gout according to the location of crystal deposits: in articulars or in tendons. METHODS: A total of 105 patients with gout who were continuously treated in the Department of Rheumatology and Immunology of Peking University People's Hospital from June 2019 to December 2019 were selected and their knees, ankles, toes and painful joints and tendons were examined by high-frequency ultrasound. Then we grouped them according to the presence or absence of sodium urate crystals and the location of the crystals, collected their clinical data, and analyzed the clinical characteristics. RESULTS: Among the 105 patients, 25 patients had no crystal deposits in the joints or tendons (as the non-crystal group), 43 patients had intra-articular crystals (as the joint group), and 37 patients had intra-tendon crystals with or without intra-articular crystals (as the tendon group). Among them, the most involved part of sodium urate crystals deposited in the joints was the metatarsophalangeal joint (29 cases, 67.4%), followed by knee joints (10 cases, 23.2%), ankle joints (9 cases, 20.9%). The most involved part of sodium urate crystals deposited in the tendon was the quadriceps tendon (16 cases, 43.2%), followed by the Achilles tendon (13 cases, 35.1%), the patellar tendon (12 cases, 32.4%), and the three heads of brachii tendons (5 cases, 13.5%). The three groups were compared using multi-sample analysis of variance/multi-sample rank sum test. Age, age of first increase in uric acid (UA), serum glucose (Glu) level and C reactive protein (CRP) were all significantly different. After multiple comparisons, compared with the non-crystal group, age, the age of first increase in uric acid, and CRP were significantly higher in the tendon group. There was no significant difference between the non-crystal group and the joint group. There was no significant difference between the tendon group and the joint group. CONCLUSION: In patients with gout, it is common for ultrasound to find crystals deposited in joints or tendons. The most commonly affected parts include the metatarsophalangeal joint, knee joint, ankle joint, quadriceps tendon, Achilles tendon, patellar tendon, and triceps tendon. There were significant differences among the three groups in age, age of first increase in uric acid, CRP and blood glucose, and the proportion of urinary calculi in patients with crystal deposits was significantly higher than those without crystal deposits.

OBJECTIVE: To assess the diagnostic value of salivary gland ultrasonography (SGUS) for IgG4-related sialadenitis. METHODS: Ultrasonography examination of major salivary glands was conducted for 48 IgG4-related sialadenitis patients and 50 Sjögren's syndrome patients, whose ages and disease duration were matched. The imaging features were graded using two different scoring systems (0-16 and 0-48, respectively) obtained from the grades of bilateral parotid and submandibular glands. The scores were used to further evaluate the features of salivary gland ultrasonography in IgG4-related sialadenitis and to compare them with Sjögren's syndrome patients. The association of SGUS scores of IgG4-related sialadenitis group with serological tests was analyzed. RESULTS: The mean age of IgG4-related sialadenitis group and Sjögren's syndrome group was 49.23 years and 50.44 years, respectively. The serum IgG4 level of the patients in the IgG4-related sialadenitis group was increased, with an average (9.60±6.43) g/L. And the serum IgE level was at a median of 251.5 (123.4-543.6) IU/mL. In the 0-16 system, the scores of submandibular glands of the patients in IgG4-related sialadenitis and Sjögren's syndrome were 6.0 (6.0-8.0) and 4.0 (2.0-8.0), and the scores of the total four glands were 10.0 (8.0-14.0) and 8.0 (4.0-12.0) respectively. In the 0-48 system, the scores of submandibular glands with IgG4-related sialadenitis and Sjögren's syndrome were 18.0 (14.5-20.0) and 11.0 (7.0-14.0), and the scores of the total four glands were 26.0 (18.5-34.0) and 21.5 (15.0-26.3) respectively. It suggested that in the 0-16 system and the 0-48 system, scores of submandibular glands and the total of four glands of IgG4-related sialadenitis were higher than those of Sjögren's syndrome. Meanwhile, the association analysis of 0-48 system showed a positive correlation of SGUS scores with serum IgG4, which also showed a positive correlation of SGUS scores with serum IgE in 0-16 system. CONCLUSION: Semi-quantitative ultrasonography scoring systems can evaluate and quantify the lesions of salivary glands, which can be helpful in the diagnosis and differential diagnosis of IgG4-related sialadenitis combined with the clinical manifestations, serological indicators and/or histopathological manifestations. Ultrasonography can also assess the activity of IgG4-related sialadenitis preliminarily.

ABSTRACT

OBJECTIVE: To investigate the changes of bone mineral density (BMD) and serum bone turnover factor in newly diagnosed systemic lupus erythematous (SLE) patients. METHODS: Eighty newly diagnosed SLE patients and 80 age and gender matched healthy controls were enrolled. None of the SLE patients had ever received glucocorticoid, immunosuppressive agents or vitamin D. BMD was measured at radius,lumbar spine and hip by dual X ray absorptiometry (DXA). Bone turnover markers including serum levels of tartrate-resistant acid phosphatase 5b (TRAP5b),bone alkaline phosphatase (BAP) and 25-hydroxy vitamin D3 (25-OH-VD3) were measured by enzyme-linked immunosorbent assay (ELISA). Logistic regression was employed to analyze the risk factors associated with decreased BMD. RESULTS: Mean age of the SLE patients was (32.8±12.4) years, and 85% were female, none of whom were post-menopausal. BMD was significantly reduced in all the measured sites, compared with the healthy controls. Sixteen (20%) of the patients were osteopenic in at least one site measured locations. The serum levels of 25-OH-VD3 were markedly reduced in the newly diagnosed SLE patients than those of the normal controls [(46.1+12.3) nmol/L vs. (25.4+11.2) nmol/L, P<0.001)]. The serum levels of 25-OH-VD3 in the SLE patients with nephritis were much lower than those without nephritis (P=0.04). A significant negative correlation was demonstrated between the serum concentration of 25-OH-VD3 and the disease activity scores as measured by SLE disease activity index (SLEDAI) (r=-0.3,P=0.001). The serum TRAP5b concentration was positively correlated with SLEDAI (r=0.435,P=0.003). Age (P=0.058) and SLEDAI (P=0.085) were probably associated with decreased BMD in Logistic regression analysis. CONCLUSION: The study showed reduced BMD in untreated SLE patients. The role of chronic inflammation was of probable importance in bone metabolism.

ABSTRACT

Objective: To evaluate the safety and efficacy of ombitasvir/paritaprevir/ritonavir (OBV/PTV/r) 25/150/100 mg once daily combined with dasabuvir 250mg, twice daily in non-cirrhotic Chinese adult patients with newly diagnosed and treated chronic HCV genotype 1b infection. Methods: A randomized, double-blind, placebo-controlled, multicenter phase 3 clinical trial was conducted in mainland China, Korea, and Taiwan.Safety and efficacy of OBV/PTV/r plus DSV administered for 12 weeks were evaluated in a newly diagnosed and treated (interferon alpha /pegylated interferon alpha) and ribavirin non-cirrhotic adults with chronic HCVgenotype 1b infection. Patients randomly received OBV/PTV/r plus DSV for 12 weeks (Group A), or placebo for 12 weeks (Group B) followed by an open-label phase of OBV/PTV/r plus DSV for 12 weeks. Sustained response (SVR12) rate obtained at 12 weeks and (SVR24) 24 weeks after discontinuation of treatment, and the incidence of adverse events and laboratory abnormalities after double-blind and open-label phase treatment were assessed. Results: A total of 410 cases of Chinese patients were included and randomly assigned to group A and B (with 205 cases in each group) in a 1:1 ratio. The rates of SVR12 and SVR24 were 99% (95% CI: 94.8% - 99.8%) in the newly diagnosed patients in group A (205 patients) and the rates of SVR12 and SVR24 were 100% in treated patients (95% CI: 96.3% - 100%). Different baseline characteristics had no effect on SVR12 and SVR24 rates. Most of the adverse events occurred were mild, asymptomatic, and≥ 3 laboratory abnormalities during treatment were rare, including elevation of alanine aminotransferase (2 cases in double-blind stage A group), aspartate aminotransferase (Double-blind stage A (3 cases) and total bilirubin (1 case in open-label phase B group); however, those mild adverse events could be recovered after drug withdrawal or discontinuation. only1 person discontinued drugs due to adverse events (Group B, open-label phase). Conclusion: The 12 weeks treatment course of OBV/PTV/r combined with DSV produced 99% ~ 100% rates of SVR12 and SVR24 in non-cirrhotic Asian adult patients with newly diagnosed and treated chronic HCV genotype 1b infection, and the tolerance and safety were good.

OBJECTIVE: To investigate the effects of miR-519d on the proliferation, cycle and invasion of human prostate cancer PC3 cells and its possible molecular mechanism. MATERIALS AND METHODS: The proliferation, cycle, and invasion of human prostate cancer PC3 cells were detected via cell counting kit-8 (CCK-8) and transwell assay. The expression of NRBP1 mRNA was detected via reverse transcription-polymerase chain reaction (RT-PCR). Western blotting was used to detect the expression of NRBP1, cyclin D1, and epithelial-mesenchymal transition (EMT) markers. RESULTS: The expression of miR-519d in prostate cancer was decreased, which was correlated with tumor size, metastasis, and staging. Proliferation, cycle, and invasion of PC3 cells were significantly decreased after overexpression of miR-519d. Bioinformatics analysis and Western blotting showed that there was a potential miR-519d binding site in NRBP1 3'-UTR, and overexpression of miR-519d significantly inhibited the expression of NRBP1. The expression of E-cadherin in PC3 cells overexpressing miR-519d was up-regulated, and the expressions of N-cadherin, cyclin D1, vimentin, fibronectin, and Snail were down-regulated. CONCLUSIONS: MiR-519d can repress the proliferation, cycle, and invasion of prostate cancer PC3 cells by inhibiting NRBP1.

ABSTRACT

Objectives: To observe the correlation between the different ultrasonic manifestations of gouty arthritis and the indexes of bone destruction (Dickkopf-1, RANKL), in order to further explore the role of musculoskeletal ultrasonography in the diagnosis and monitoring of gouty arthritis. Methods: 160 cases of gouty arthritis from clinic of the Department of Rheumatology of Peking University People's Hospital between July 2016 and June 2017 were recruited in this study. These patients were examined by musculoskeletal ultrasonography (bilateral first metatarsophalangeal joints, bilateral ankle joints and bilateral knee joints). Grouping was based on ultrasound features of joints. Group A showed no aggregate .Group B showed aggregate and double contour signs. Group C showed tophus and bone erosion. Serum concentrations of Dickkopf-1 and RANKL were measured. The correlations of Dickkopf-1 or RANKL and clinical/laboratory parameters were analyzed. Results: (1)There was a significant difference in Dickkopf-1 concentration between the three groups(P<0.001). And the group C[(1 722.2±482.7)ng/L]was higher than the group B[(1 309.3±496.4)ng/L](t=4.418, P<0.001), and the group B was higher than group A[(807.9±373.8)ng/L](t=6.137, P<0.001). (2)There was a significant difference in RANKL concentration between the three groups (P<0.001). And the group C[(0.78±0.47)ng/L]was higher than the group B[(0.35±0.29)ng/L](t=5.456, P<0.001), and the group B was higher than group A[(0.10±0.09)ng/L](t=6.923, P<0.001). (3)The level of Dickkopf-1 was associated with the disease duration(r=0.430, P<0.001), and the level of RANKL was associated with the disease duration(r=0.359, P<0.001). Conclusion: Aggregates, double contour signs, tophus and bone erosion can be observed in the ultrasonic examination of gouty arthritis. And the longer the duration of the disease, the more likely the extent of bone destruction is. Joint ultrasound can be the first imaging examination for the extent of joint involvement in gouty arthritis. And the longer the duration of the disease, the more likely the extent of bone destruction is. Musculoskeletal ultrasonography can be the preferred imaging method for detecting the extent of joint involvement in gouty arthritis.

ABSTRACT

This study examined the attitudes of scientists in Taiwan's leading animal research institution toward xenotransplantation. The aim was primarily to evaluate the opinions of professionals in the biomedical field on key issues including ethical moral, legal, and regulatory issues raised by the biotechnology. A secondary objective was to identify potential factors that influenced opinions. A questionnaire-based survey was used to evaluate opinions. A test for internal consistency of the questionnaires to sample of 91 scientists was performed as well as a principal component analysis. We evaluated associations between variables using the nonparametric Kruskal-Wallis test. Among the subjects 85.2% thought that xenotransplantation can be more beneficial than harmful to human society and 94.3% believed that it is important to develop xenotransplantation. Also, 97.8% of participants believed that legislative guidelines should be adopted to regulate research in biotechnology. Gender was an influencing factor, whereas, variables such as religion, marital status, and age did not have obvious effects. Further studies on the general public are needed to detect other factors and to examine the attitude of nonprofessionals toward xenotransplantation.

ABSTRACT

Polyetheretherketone-hydroxyapatite composites were developed as alternative materials for load-bearing orthopedic applications. The amount of hydroxyapatite (HA) incorporated into the polyetheretherketone (PEEK) polymer matrix ranges from 5 to 40 vol% and these materials were successfully fabricated by injection molding. This study presents the mechanical and biological behavior of the composite materials developed. It was found that the amount of HA in the composite influenced the tensile properties. Dynamic behavior under tension-tension fatigue revealed that the fatigue-life of PEEK-HA composites were dependent on the HA content as well as the applied load. The biological responses of PEEK-HA composites carried out in vivo verified the biocompatibility and bioactive nature of the composite materials.

ABSTRACT

INTRODUCTION: Myasthenia gravis is an autoimmune disease characterized by the presence of circulating IgG antibodies, which interact with acetylcholine receptors and interfere with neuromuscular transmission. OBJECTIVES: To compare neuromuscular function when mivacurium is used in patients with myasthenia gravis and patients with no impairment of transmission at the neuromuscular synapse. MATERIAL AND METHOD: Prospective study of 40 patients in two groups. Group I (n = 20) consisted of patients with no impairment of neuromuscular transmission who underwent sternotomy or mediastinoscopy and who received 2 ED95 of mivacurium. Group II (n = 20) were patients with myasthenia gravis who underwent transsternal thymectomy and received 0.5 ED95 of mivacurium (50 micrograms/Kg). The neuromuscular function of all patients was monitored by accelerometry of the thumb adductor. RESULTS: All demographic variables except sex were similar in the two groups. Time to maximal block, duration of block and the recovery at T1 25-75 were significantly greater in group II (250 +/- 10 s, 29.1 +/- 2.4 min and 8.1 +/- 1.5 min, respectively) than in group I (188 +/- 13 s, 21.2 +/- 0.4 min and 7.1 +/- 0.2 min in group I). Maintenance doses were given more often in group I. At the end of surgery and before recovery from mivacurium, the mean twitch height in group II was 89.3 +/- 0.5%, such that tracheal tubes were removed from 95% of the patients without complications. The mean time until extubation in group II was 17.8 +/- 1.3 min and was related to the extension of the blocks. The differences were statistically significant (p < 0.05). CONCLUSIONS: The anesthetic effect of mivacurium was twice as great in myasthenic patients, in whom it behaved like an intermediate-level non-depolarizing muscle relaxant. Mivacurium can reduce prolonged mechanical ventilation in patients who are myasthenic or pharmacologically immunosuppressed and at risk of sepsis arising in the respiratory tract.

Introducción: La miastenia gravis es una enfermedad autoinmune que se caracteriza por presentar anticuerpos circulantes del tipo IgG. Estos anticuerpos interactúan con los receptores colinérgicos e interfieren en el mecanismo de la transmisión neuromuscular. Objetivos: Identificar las diferencias en la respuesta a la monitorización de la función neuromuscular con el empleo de mivacurio en pacientes miasténicos y compararlo con los resultados de los pacientes sin afección de la placa neuromuscular. Material y Métodos: Estudio prospectivo en 40 pacientes, divididos en dos grupos. El grupo I (n = 20) fueron pacientes sin afecciones de la placa neuromuscular a los que se les practicó esternotomías o mediastinoscopías y recibieron 2ED95 de mivacurio. El grupo II (n = 20) eran pacientes con miastenia gravis a los cuales se les realizó timectomía transesternal y se les administró 0,5 ED95 de mivacurio (50 mg/Kg).A todos se les monitorizó la función neuromuscular con aceloremetría del aductor del pulgar. Resultados: Las variables demográficas fueron homogéneas en ambos grupos a excepción del sexo. Los tiempos hasta el bloqueo máximo, de duración clínica, y el índice de recuperación T1 25-75 fueron significativamente mayores en el grupo II: 250 ñ 10 s, 29,1 ñ 2,4 min y 8,1 ñ 1,5 min, respectivamente, y 188 ñ13 s, 21,2 ñ 0,4 min y 7,1 ñ 0,2 min en el grupo I. Las dosis de mantenimiento fueron más frecuentes en este grupo. Al finalizar el acto quirúrgico y sin descurarizar los pacientes, los valores medios de la altura del twitch en el grupo II fué de 89,3 ñ 0,5 por ciento, por lo que el 95 por ciento de los pacientes fueron extubados sin complicaciones. La extensión del bloqueo en estos pacientes condicionó que la media del tiempo que tardaron para extubarse la totalidad del grupo II ascendiera a 17,8 ñ 1,3 min. Estas diferencias resultaron ser estadísticamente significativas (p<0,05). Conclusiones: El mivacurio resultó tener una eficacia clínica 2 veces mayor en el grupo II. Éste se comportó como un bloqueante relajante no despolarizante de acción intermedia en los miasténicos. La prolongación de la ventilación mecánica en los pacientes miasténicos, inmunodeprimidos farmacológicamente y con riesgo de sepsis de origen respiratorio puede ser minimizada con este bloqueante neuromuscular. ABSTRACT

INTRODUCTION: We report the first case of hard metal lung disease in Singapore and the occupational investigative work and control measures that were undertaken. CLINICAL PICTURE: A 38-year-old machinist in the tool manufacturing industry presented with exertional dyspnoea and cough. Chest X-ray revealed bilateral reticulonodular infiltrates with honeycombing. High resolution computed tomography scan of the thorax confirmed the presence of interstitial fibrosis. Open biopsy of the lung showed features of pneumoconiosis. Particle induced X-ray emission (PIXE) analysis, a relatively new elemental analysis technique, performed on the lung biopsy specimen confirmed the presence of tungsten and titanium; and he was diagnosed to have hard metal lung disease. Microbiologic, serologic and histologic investigations excluded an infective cause. Serial pulmonary function tests on follow-up showed no progression. He presented with haemoptysis 10 months later and was diagnosed to have tuberculosis on the basis of positive sputum and bronchoalveolar lavage cultures for Mycobacterium tuberculosis complex. TREATMENT: Preventive measures and permanent transfer to non-cobalt work were instituted. OUTCOME: The interstitial fibrosis appears to have stabilised. CONCLUSION: The diagnosis of hard metal lung disease must be considered in a worker exposed to cobalt presenting with interstitial fibrosis.

ABSTRACT

An insertion sequence element, ISXC6, was isolated from Xanthomonas campestris pv. campestris 17 (Xc17). Sequence analysis showed that it is 1,500 bp long and has 20-bp perfect inverted repeat ends. Upon transposition, a direct repeat TAATTC was generated, flanking this IS. No significant homology was observed between this sequence and other sequences in database. Results of Southern hybridization showed that multiple copies of ISXC6 were present in 7 strains of Xanthomonas examined.

In an attempt to investigate the role of left ventricular blood outflow in the generation of systolic anterior motion (SAM) of the mitral valve in patients with hypertrophic cardiomyopathy, we precisely analyzed the temporal relation of SAM and the left ventricular outflow tract (LVOT) systolic Doppler events obtained at the maximal mitral-septal apposition or equivalent area in eight patients with severe SAM, in five patients with mild/moderate SAM, and in seven patients with no SAM, using M-mode and pulsed Doppler echocardiography; the results were compared with those in 10 normal subjects. In all 13 patients with SAM, the timing of SAM generation corresponded to the LVOT Doppler events either between the onset of SAM and the onset of Doppler (r = 0.834, p less than 0.0001) or between the peak of SAM and the peak of Doppler (r = 0.836, p less than 0.0001). The excursion rate of the development of SAM showed a correlation with the LVOT blood outflow acceleration (r = 0.828, p less than 0.0001). The timing of SAM resolution also correlated with the Doppler events, either between the offset of SAM and the offset of Doppler (r = 0.795, p less than 0.001) or the end of SAM and the end of Doppler (r = 0.859, p less than 0.0001). The LVOT blood outflow deceleration showed a correlation with the regression rate of SAM (r = 0.668, p less than 0.013). The LVOT blood outflow acceleration was significantly higher in patients with severe SAM than in patients with mild/moderate SAM or no SAM. This study suggests that the high LVOT blood outflow acceleration in early systole possibly plays an important part in the generation of the Bernoulli pressure drop and results in anterior motion of the mitral valve. At mid-systole, a drag force and/or suction effect of pressure drop produced by continuous outflow blood may sustain the anterior motion of the mitral valve. At late systole, as the blood flow decelerates, the regression of SAM then occurs.

ABSTRACT

The concentration of zinc and copper in the hair of 55 patients with epilepsy was determined. The hair copper levels in patients with epilepsy were significantly lower than those in the normal control group (P less than 0.001). The hair zinc levels in female patients were significantly lower than those in the normal control group (P less than 0.05). In the following patients: (1) with generalized seizure, (2) with the disease course 0-5 years, (3) EEG showed heavy or middle abnormality, (4) had epileptic attacks within one year before the sample was taken, the zinc levels were also significantly lower compared with controls (P less than 0.05). Antiepileptic drugs had more effect on hair zinc than on copper. The increase in the hair zinc and copper and zinc: copper ratio seems to show the efficiency of drugs.

ABSTRACT

In this paper, regression models with error terms generated by lower order ARMA schemes are analyzed. Methods are discussed for estimating the parameters of the regression coefficients and the ARMA processes. The problem of detecting changes in the regression parameters is considered. A change-detection statistic proposed by MacNeill (1978) for regression problems is modified for application to ARMA processes. The effect of autocorrelated errors on this statistic is briefly discussed.

ABSTRACT

The Sr/Ca ratios in molluscan shells in Singapore waters were measured using the X-ray fluorescence (XRF) technique. Our results show that the Sr/Ca ratio varies greatly among different species. However, within the same species, this ratio is practically the same for samples collected from sites close together but varies significantly for samples from sites far apart. Furthermore, this study shows that whole shell analysis using the XRF technique is simple and quick and that its application to environmental monitoring seems feasible.
